Reframing the role and impact of pharmacy in chronic disease care

Chronic non-communicable diseases (NCDs), such as cancer, chronic respiratory diseases, cardiovascular diseases and diabetes represent a worldwide health challenge. According to the WHO 41 million people die each year from NCDs, accounting for 74% of all deaths globally. Prevention and management of chronic diseases is dependent on reducing underlying risk factors. Community pharmacists and their teams are ideally placed to raise chronic diseases awareness, identify those at risk and be the driving force behind earlier detection and referral to primary care partners for screening, diagnosis and intervention. At this event, the “Chronic Disease Service Framework” to support pharmacists in providing a range of NCD pharmacy services was presented.
